How Do I Enroll My Child?
•A parent or guardian must complete and sign a permission form to enroll each child under 18 years of age
•Students 18 or older can register themselves
What is the Cost?
•CIFC Health will bill Medicaid/HUSKY or insurance companies directly for eligible services. Uninsured patients are also eligible for care on a sliding fee scale.
•For specific questions regarding services, costs, or use of a School Based Health Center, please reach out to the Health Center at your child's school.
